Hospice at home nursing service

Our nurses work in collaboration with GPs, Macmillan nurses and District nurses.

What is the service ?

This is a new, developing service providing nursing care and support for patients and carers at home.

Currently the nurses are working with 3 GP practices in St. Albans - The Lodge Surgery, The Maltings Surgery and Parkway House Surgery and the 3 GP surgeries in Harpenden - Davenport House Surgery, The Elms Medical Practice and the Village Surgery.

Access to nursing care is available 24 per day, 7 days a week.

Who is the service for ?

The service is for patients who have specialist palliative/complex nursing needs and wish to remain at home as far as possible. The needs of the patient and family/carers are such that additional supportive care is required, including at end of life.

How do you access the service ?

Your GP's consent is required for referral.
