Hospice at home nursing service
Our nurses work in collaboration with GPs, Macmillan nurses and District nurses.
What is the service?
The service provides nursing care and support for patients and carers at home, 24 hours a day, 7 days a week. It is free of charge.
Who is the service for?
The service is for patients who have specialist palliative/complex nursing needs and wish to remain at home as far as possible. The needs of the patient and family/carers are such that additional supportive care is required, including at end of life.
How do you access the service?
Any patient or carer registered with a GP in St Albans or Harpenden can access the Hospice at Home service, subject to agreement from their GP. For more information about Hospice at Home and the process of referral please contact us on 01727 731001.
